Unified Program Consolidated Forms (UPCF)
The following forms have been developed for use by businesses to submit their Hazardous Materials Release Response Plans and Inventory (Business Plans), which satisfy the California Fire Code Hazardous Materials Management Plan/Hazardous Materials Inventory Statement (HMMP/HMIS) requirements:

The Certified Unified Program Agencies (CUPA) determine what forms to use to submit the remaining required Business Plan elements:
- Emergency Response Plans and Procedures,
- Training Program information, and
- Additional information- contact your local city or county CUPA to determine if additional information is required.
Note:- All regulated businesses and local governments will be required to submit their regulatory reports, such as the HMMP/HMIS and Business Plan data, electronically by 2013. The regulated community will be able to report through the California Environmental Protection Agency (Cal/EPA) electronic reporting system or directly to their local Unified Program Agency's electronic portal.
